Immunotherapy has revolutionized the treatment of cancer and autoimmune diseases, yet significant challenges remain in translating preclinical discoveries to clinical practice. This comprehensive review examines the current landscape of immunotherapeutic approaches, including immune checkpoint inhibitors, CAR-T cell therapy, and cancer vaccines. We analyze the critical barriers to successful translation, including tumor heterogeneity, immune evasion mechanisms, and the need for predictive biomarkers. Recent advances in combination therapies and personalized approaches are discussed, along with emerging technologies such as neoantigen prediction and oncolytic viruses. The review concludes with a framework for accelerating the bench-to-bedside translation of immunotherapeutic innovations, emphasizing the importance of interdisciplinary collaboration, standardized preclinical models, and patient-centric trial design.
